The Ontario Student Record (OSR) is a folder that contains important documents about each student that all schools must have in a secure place as per the Ministry of Education. Some of the following documents will be required by the student to send to Clyfford Academy by mail.

1) Biographical Data ( name, date of birth) – can be a photocopy of a passport
2) Schools Attended ( official Transcripts)
3) Parent/ Guardian Information ( name, date of birth, place of work, emergency contact info)
4) Health information ( Immunization record)
5) Report Cards ( provided by Clyfford Academy)
6) Prior Learning and Equivalency Assessment ( if applicable, provided by Clyfford Academy)
7) Substitution Forms ( if applicable, Provided by Clyfford Academy)
